Your First Visit
Dr David Shaker will help you through the investigations and management of your cause for infertility with great care and sensitivity. Dr Shaker consults from his clinic at the Mater Medical Centre and works with the staff at Monash IVF Rockhampton.
How do I set an appointment with you?
Contact Dr Shaker’s rooms to make an appointment. In order to obtain a rebate from Medicare for your first visit and any ongoing fertility care, you will need to have a referral from your GP or another specialist.
Your GP may give you a 12 month or an indefinite referral. A referral from another specialist will last for only 3 months from the date of your first consultation. If you are seeking medical advice as a couple, it is a Medicare requirement that the names and dates of birth of both partners appear on referrals.
What do I need to bring to my appointment?
Please bring your referral (even if this has already been sent to the Doctor), your Medicare card and any Private Health Insurance cards or details.
If you have begun any infertility investigations with your GP or another specialist, please bring along any test results or paperwork.
